ABC recently released this synopsis of the episode:
The inhabitants of Storybrooke brace themselves for the end when Greg and Tamara detonate the trigger Regina had placed within the curse, with the annihilation of the town and its residents imminent; and Mr. Gold mourns the loss of his son, Bae/Neal. Meanwhile, back in Neverland of the past, Hook discovers his connection to a young Bae after he rescues him from the sea and soon realizes that the Lost Boys are in hot pursuit of the boy.
This sneak peek of the episode shows Greg and Tamara really and truly activating the trigger. While we have a hard time imagining the show without Storybrooke, the cast and crew have hinted that the finale is totally game changing and extremely shocking, so it's possible we really will say goodbye to Storybrooke.
Meanwhile, we're also very interested to learn more about Hook and Bae's backstory. The hint that Hook "discovers his connection" to Bae suggests he realizes that Bae is Milah's child (that might explain this photo; maybe Bae discovers a sketch of Milah in Hook's things). And the clue that the Lost Boys are "in hot pursuit" of Bae suggests that Hook may be the (relative) good guy in this situation, at least from Bae's point of view.
