Original story (February 25, 2014): Two of the individuals involved in a fatal February 23 car accident in Los Angeles had connections to Hollywood. Sami Hayek, who survived, is the younger brother of actress Salma Hayek; and Ian Cuttler Sala, who died, was a Grammy-winning photographer and art director.

Worse yet, the coroner believes Sami, 40, might be to blame for the wreck, as People reports.

Los Angeles County Coroner's Lieutenant Fred Corral tells the magazine that Sami was "apparently speeding" in his 2006 Ford GT on Sunset Boulevard that afternoon before losing control and swerving into oncoming traffic, striking a pickup truck.

Sami and the driver of the other vehicle sustained broken bones and other injuries, but Ian — a passenger in Sami's car — lost his life. The lieutenant notes that Ian was wearing a seat belt.

The Los Angeles People Department, as People notes, has not yet concluded whether Sami was indeed speeding.

According to Ian's official site, the 43-year-old had created visual campaigns for artists like Beyonce, Ricky Martin, and Mariah Carey during his nine-year career at Sony Music. He won a Grammy in 2005 for his art direction of the Johnny Cash: The Legend box set. He later devoted his career to photography full-time.

Update (February 26, 2014): Beyonce took to Facebook on February 25 to pay her respects to Ian and to elaborate on their collaboration. "Rest in peace, Ian Cuttler," she wrote. "Ian was a Grammy Award-winning art director and photographer who will be greatly missed. His memorable work includes album covers for Destiny Fulfilled and Dangerously in Love."
