On this week's episode we learned the struggling Liar is receiving some sort of "treatment," and it's important enough she kept it up even now that she's in Rosewood. Also, she "really need[s] [them] to work."
So, why exactly is Em popping pills and getting needles stuck in her? We have some ideas.

Cancer. It's TV: Half the time someone is sick, it's cancer. This certainly seems like the easiest way for the writers to go if they want to give her something life threatening, but possible to recover from. Still, would she really keep this from her mom and friends?
An infection caught doing something risky. On the other hand, perhaps she's fighting off some rare infection, maybe caught while doing something risky, or while traveling while pretending to be in school.
Em is doing her best to pretend her life is just fine while really, it's spinning out of control, so this is a secret it might make more sense to keep.

She wants to donate eggs. Going in a completely different direction, perhaps her treatments aren't because she has an illness, but because she's preparing to donate eggs. We know she's strapped for cash, and hiding this kind of secret from her mom seems more reasonable than keeping a life-threatening illness under wraps.
We hope the writers don't draw this mystery out for too long, because we're really worried!
