It’s very possible Jack finds himself in the wrong place at the wrong time and tries to play the hero in a robbery or some sort of attack. This could then lead to him being shot to death, though it doesn’t explain how Kate’s involved.

This is certainly the most disturbing and emotional way for Jack to die, but it’s not out of the realm of possibility, especially considering the state of his marriage at the end of Season 1.
